New Patients
We welcome new patients seeking help with chronic pain conditions. Here's everything you need to know before your first visit.
Getting Started
All visits are by appointment only. You may schedule an appointment without a referral, but a physician referral is required before you can be seen.
You may schedule an appointment before your referral is received. However, we must receive a referral from your primary care provider before you can be seen.
Please ask your primary care provider's office to fax the referral to:

Patient Portal
For patient privacy and compliance with HIPAA regulations, Restore Pain Center uses a secure patient portal through OptiMantra for all medical communication and documentation.
Portal access is available to established patients who have been entered into the system and have a scheduled appointment. If you are a new patient, please call or email us to get started — we will set up your account when your appointment is confirmed.
Once your account is active, you will receive login instructions to access your secure OptiMantra portal.
Save time: Patients are strongly encouraged to complete their intake forms through the patient portal before arriving. If forms are not completed in advance, please plan to arrive at least 20 minutes early.

If assistance is needed completing forms, please call our office and we will be happy to help.
Payment is made at the time of appointment. For initial appointments and most follow-up visits, a Urine Drug Screen (UDS — 10 panel urinalysis) will be required. Once the UDS has begun, intake is in process and no refunds are given at that point.
