All visits are by appointment only. New patients need an established chronic pain diagnosis and a medical referral before they can be seen. You may request an appointment before the referral arrives.

1Talk with your current provider about whether referral-based pain management is appropriate for your condition.
2Ask the referring office to send the referral and relevant records to Restore Pain Center by fax at 601-510-5268.
3Use the secure appointment-request form to begin scheduling, then bring available records and medication information to your visit.
